Our Madisonville clinic is our main office and our largest clinic.  This is a brand new facility that we designed and built from the ground up to be a team oriented, patient-centered medical home with an emphasis on accessibility, convenience, and efficiency.  

Our Tellico Plains clinic is conveniently located near Highway 68.   Chota Community Health Services was made the caretaker of this historic building, and we've put a lot of time and care into making it truly worthy of the wonderful community that we serve.

Our Vonore clinic is located directly off Highway 411 in the city of Vonore.  Its convenience, ease of access, and comfortable design has made it a great asset to those in the Vonore area.

Through a unique partnership with the Monroe County School System, Chota Community Health Services is proud to offer healthcare services directly within our county schools.  CCHS provides support to allow staffing every school with a nurse, and also staffs Nurse Practitioners in the schools to provide medical services to students and staff.
